Cognition and brain volumes in late-life depression.
Depression is one of the most common and disabling of psychiatric disorders, especially in late life. Relatively little is known about the neurobiology of late-life depression, and its clinical features are poorly understood. The purpose of this study was to use Magnetic Resonance Imaging (MRI) to measure differences in the volume of relevant brain regions in elderly patients with and without late-life depression. Twenty-two patients with major depression and 22 healthy controls (HC) were studied with a 3T MR scanner. A manual segmentation procedure was used to measure the volumes of the hippocampus, thalamus, amygdala, insula, putamen, and frontal cortex. White matter hyperintensities (WMH) were rated visually on T2-weighted MRI scans. The volume of the amygdala, insula, putamen, and frontal cortex were significantly smaller in patients with major depression than in HC. The volume of the right hippocampus and amygdala, but not the volume of the left hippocampus or thalamus, were correlated with depression severity as measured by the Hamilton Depression Rating Scale. WMH did not significantly differ between patients and HC and did not correlate with any brain volumes. These results provide the first MRI evidence that major depression is associated with structural alterations in the limbic-cortical network, which may be related to the pathophysiology of late-life depression.About Me
